Comments:
A renewal inspection was initiated on July 12, 2022 and concluded on July 13, 2022. There were 45 children present, ranging in ages from infancy to school aged, with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 4 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of staff record and interview, the center did not ensure to obtain a repeat central registry search every five years.
Evidence: In Staff#2's record (date of hire: 01/23/17), the central registry search results was dated 02/02/17. Interview with staff confirmed this was the most recent background checks available for review during this inspection.

The center submitted staff background check request to obtain results. The center will ensure updated check results are included in staff record for review.

Based on review of documentation and interview, the center did not ensure to obtain an annual approval from the health department as required.
Evidence: The health inspection available for review during the inspection was dated 02/16/21. Interview with staff confirmed the documentation available for review was most recent.
Plan of Correction:
The center contacted health department to schedule annual appointment and is scheduled for 07/22/22.
